1. Kaiser Permanente Vendor Code of Conduct Compliance

 

Kaiser asks vendors to disclose actual, potential, or perceived conflicts of interest. In keeping with this request O3, Inc. hereby discloses that Officers of the company have family members that are employed by Kaiser Permanente. These individuals are not employed by nor do they receive compensation from O3, Inc. but are beneficiaries of the solutions O3, Inc. provides to Kaiser as a whole.
